S*T*A*R Series Forms

MODIFY YOUR RESERVATION

Reminder: All tickets are priced at $5 per person. Palm Beach County students eligible for the Federal Free/Reduced Lunch Program qualify for waived admission. Those students can be noted as “students on waivers” in the form below.

For Title I schools, all students are considered students on waivers.

This form will allow you to confirm/update your headcount and indicate Title I status
OR cancel your S*T*A*R Series reservation.*

*Cancellations must be made two weeks prior to the day of the performance. A refund will not be issued if your order is cancelled within the two-week time frame. Cancellations are final once the request is submitted.

ACCESSIBILITY FORM

Please use this form to indicate the accessibility needs of any student(s) attending a S*T*A*R Series performance.

• Sign language interpreters MUST BE REQUESTED by a teacher/school no later than two weeks prior to the performance.
• If a student requiring a sign language interpreter is not able to attend, please call the Education Department at 561.651.4251 as soon as possible to cancel.
• Sign language cancellation requires 72-hour notice
• Schools will be billed for cancellation fees. The Kravis Center will not be responsible for payment.
